FIELD: medicine, gynecology in particular.
SUBSTANCE: invention relates to medicine, namely to obstetrics and operative gynecology. Ligation of the uterine, ovarian and internal iliac arteries is performed on both sides. At the same time, a compression suture is applied to the posterior wall of the uterus, starting from the area of the sacro-uterine ligaments, in such a way that the first injection is made through all layers of the uterus along the posterior wall at the level of the upper third of the sacro-uterine ligaments, retreating from the vascular bundle by 0.5 cm, to the left or right - at the surgeon's choice, a needle of 40 mm is injected, not reaching 1 cm to the median line of the uterus. Next, an injection at a distance of 1 cm from the median line at the same level in the direction of the opposite vascular bundle and again an injection, not reaching it 0.5 cm. Then the ligature is tied with three knots, while the ends of the thread should be of the same length. The next suture is performed with the free ends of the threads 3 cm above the previous one, alternately on both sides, the injection is made from the median line of the uterus outward to the right and left through all layers of the uterus, the injection is made in the direction of the ribs of the uterus, without affecting the vascular bundles. The ligature is tied again with a metered effort, thereby screwing the layers of the uterus into the suture line. These sequential actions are performed to the area of the bottom of the uterus with a transition to its anterior wall, at the same time, the tone of the uterus is palpated.
EFFECT: method makes it possible to improve the quality of obstetric care and organ preservation due to reliable hemostasis in maternity patients with developed uterine hypotonic bleeding, as well as to reduce the number of radical surgical interventions (extirpation, amputation of the uterus) and preserve the reproductive function of women.
