FIELD: medicine; orthopedic dentistry.
SUBSTANCE: method is intended for use in the manufacture of complete removable dentures. The patient is taken an anatomical impression with a standard impression spoon. After making a rigid individual impression tray, its edges are adjusted in the neutral zone of the anterior and lateral sections of the oral cavity. Then, it is fitted at the distal site. To do this, first of all the degree of compliance of the oral mucosa in the glandular zones to the right and left of the blind holes, as well as their size are determined by palpation. Next, the anatomical location, the degree of mobility of the palatine curtain and the shape of the slope of the soft palate are determined with the help of nasal-inflatable and voice tests. The future border of the complete prosthesis and the shape of the glandular zones are outlined directly in the oral cavity with the help of an indelible marker. These data are used to determine the thickness, width, shape and location of the wax overlay in the area of the glandular zones and line A. The spoon is applied to the edentulous jaw, and the configuration of the glandular zones with the borders of the prosthesis applied with a marker is displayed on its inner surface. The distal edge of the spoon is shortened to the required length. A slightly heated wax overlay is applied to the designated areas of the individual spoon, strictly corresponding to the shape and outlines of the glandular zones, which is integral with the wax on the distal border of the spoon at its edge. The relief of the lining is corrected with a heated spatula. With mucosal compliance of 1.5 mm and a steep slope of the soft palate, the edge of the prosthesis is placed at the level of the blind holes, and the thickness of the wax lining should be 1 mm; with mucosal compliance of 2.5 mm and an average slope of the soft palate, the thickness of the wax lining should be 2 mm, and the edge of the prosthesis should be located overlapping the blind holes by 2 mm; with mucosal compliance of 3.5–4 mm and a gentle slope of the soft palate, the thickness of the wax lining should be 3 mm, and the edge of the prosthesis should be located overlapping the blind holes by 3–4 mm. After the final design of the distal border of the individual spoon, its fixation in the oral cavity is checked and a functional impression is taken with the help of the silicone material.
EFFECT: due to the individual functional design of the distal part of the impression tray the method allows to improve the fixation of a complete removable prosthesis of the upper jaw.
