SUBSTANCE: invention relates to methods of surgical treatment of discogenic radiculopathy and pain syndrome caused by degenerative disc pathology. Transpedicular access is used. The Jamshidi needle is used to puncture the pedicle of the S1 vertebra. The needle is passed transpedicularly parallel to the caudal endplate of the L5 vertebra to a point in the area of the endplate of the S1 vertebra at the border of the posterior and middle thirds of the intervertebral disc and located on the line connecting the inner edge of the pedicles of the L5 and S1 vertebrae. A puncture of the disc at this point is performed. A flexible conductor is inserted through the Jamshidi needle, through which a needle-cannula is inserted to the border of the nucleus pulposus for subsequent insertion of an electrode through it. The maximum depth of electrode insertion is noted, after which 6 channels are formed in the nucleus pulposus in the mode of coagulation and ablation.

EFFECT: method provides access to the disc regardless of the anatomical features of the patient, eliminates the possibility of root injury when the needle-cannula is inserted into the disc and performing nucleoplasty, eliminates the risk of progression of degenerative changes in the disc caused by puncture of the annulus due to the peculiarities of Jamshidi needle insertion.
