FIELD: medicine; cardiovascular surgery.
SUBSTANCE: invention relates to medicine, namely to cardiovascular surgery, and can be used for controlled pulmonary thromboendarterectomy in the treatment of chronic thromboembolic pulmonary hypertension. In the preoperative period, MSCT angiopulmonography of the pulmonary artery (LA) and selective angiopulmonography are performed. The data obtained for the localization of LA lesions by thrombotic masses are graphically plotted on a chart-tablet with a schematic representation of the pulmonary artery, lobar and segmental branches, reflecting the volume and degree of damage to the pulmonary artery, both on the right and on the left side. In the intraoperative period, the thrombotic masses extracted from the LA are laid out on a chart-tablet, comparing the localization of the extracted thrombotic masses with the localization of the lesion indicated on the chart-tablet. At the same time, the completeness of pulmonary thromboendarterectomy is controlled by the degree of coincidence of the initial data on localization with actually extracted thrombotic masses from both large branches and small segmental branches of the pulmonary artery.
EFFECT: method enables intraoperative assessment of the completeness of thromboendarterectomy performed in patients with chronic thromboembolic pulmonary hypertension, detection of the presence of residual thrombotic masses by comparing the localization of the extracted thrombotic masses with the localization of the lesion indicated on the chart-tablet.
