FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to endovascular surgery, and can be used for retrograde recanalisation of occluded segmental branches of pulmonary artery in patients with chronic thromboembolic pulmonary hypertension. Method involves catheterisation of pulmonary artery branches, arterial angiopulmonography, introduction of a catheter into the mouth of the target artery through an introducer inserted in the femoral vein, retrograde contrast enhancement of the periphery of the occluded segmental pulmonary artery branches. That is followed by catheterisation of an adjacent artery collateralising the target artery, through which a retrograde passage of the coronary conductor into the postocclusal bed of the recanalised artery is performed. Microcatheter is brought along the coronary conductor through the collateral artery, which is brought to the distal part of the occlusion; control angiography is performed. Microcatheter is approached to the distal occlusion cover; hydrodynamic recanalisation is performed by introducing a contrast agent in an amount of at least 2 ml inside the occlusion. Coronary conductor is externalized from the microcatheter through the occlusion area into the guide catheter placed in the orifice of the target artery. Balloon catheter is brought on externalized coronary guide in guide catheter.
EFFECT: method enables normalizing pressure in the pulmonary artery system by recanalising the segmental branches of the pulmonary arteries regardless of the presence of a proximal stump of the occluded segmental branch.
