FIELD: medicine, ophthalmology.
SUBSTANCE: suture fixation of a three-part intraocular lens (IOL) is performed by suturing it to the iris. Intraoperatively under application anesthesia, without dilating the pupil, corneal paracentesis is performed, rotating the knife in the paracentesis lumen by 10–15 degrees and pressing it on the lower lip of the paracentesis, the anterior chamber is emptied by one third. Then, viscoelastic is introduced into the anterior chamber and the dislocated IOL is centered using a rotary hook. Next, the pupillary edge of the iris is progressively displaced to the periphery by 2–3 mm with a rotational hook in four quadrants, determining the position of the haptic elements of the intraocular lens under the iris, the needle and thread are inserted through the paracentesis, the iris is pierced, retreating 1.5–2.0 mm from the pupillary edge. Then, the front sheet of the lens capsule is pierced and, passing under one of the haptic elements of the IOL, the needle is withdrawn, piercing the iris and cornea with the needle sequentially until the place where the needle passes into the thread is between the iris and the cornea, without completely removing the needle, it is required to go over the iris, repeatedly, passing directly under the same haptic element, a stitch is formed around it, creating a “loop-noose” on the haptic element. The needle is removed through the same paracentesis, cut off from the thread, pulling the thread, a double knot is formed from its ends inside the paracentesis. The specified sequence of actions with a needle and thread is repeated from the opposite side, having previously performed paracentesis in the projection zone of the second haptic element. Next, the viscoelastic is removed from the anterior chamber by washing, the paracentesis is hydrated with a balanced saline solution, and at the end of the operation, a subconjunctival injection of dexamethasone and an antibiotic is performed.
EFFECT: method makes it possible to improve uncorrected visual acuity, obtain reliable fixation of the intraocular lens, a quick rehabilitation period and improve the patient's quality of life.
