FIELD: medicine; traumatology; orthopaedics.
SUBSTANCE: invention can be used to prevent asymmetric jamming of a cementless acetabular component when it is installed in conditions of a lower marginal osteophyte of the acetabulum. The insufficiency of the depth of immersion of the acetabular component into the acetabular cavity is determined by comparing, before impaction, the immersion of the acetabular component into the acetabular cavity with the immersion of the resection cutter into the acetabular cavity, which are placed at one angle to the horizon, which can be assessed by comparing the angles between the line connecting the upper anterior spines of the pelvic bone, and the longitudinal axes of the instruments holding the cutter and the acetabular component. If the sector of non-coverage of the acetabular component by the upper wall of the acetabulum exceeds by more than 20 degrees the sector of non-coverage by this wall of the corresponding resection cutter, the depth of immersion of the acetabular component into the acetabular cavity is insufficient. In case of insufficient depth of immersion of the acetabular component into the acetabular cavity, before the final installation of the acetabular component, the lower marginal osteophyte of the acetabulum is processed with a resection cutter. In this case, the diameter of the cutter processing the osteophyte should be 4 mm smaller than the cutter that matches the diameter of the acetabular component. The criterion for sufficient resection modelling of the lower marginal osteophyte of the acetabulum is a decrease in the difference between the non-coverage of the upper wall of the acetabulum of the resection cutter corresponding to the acetabular component and the acetabular component itself before impaction to 20 degrees. After modelling, the acetabular component is wedged in the acetabular cavity and the parts of the lower marginal osteophyte protruding beyond the acetabular component are resected.
EFFECT: method provides prevention of asymmetric jamming of a cementless acetabular component when it is installed under conditions of lower marginal osteophyte of the acetabulum and prevents possible subsequent fracture of the acetabulum due to objective determination of insufficient depth of immersion of the acetabular component into the acetabular cavity and optimal selection of a cutter for processing the lower marginal osteophyte.
