FIELD: medicine; bariatric, endoscopic surgery.

SUBSTANCE: internal opening of the fistulous tract is preliminarily visualized, fistulography of the fistulous tract is performed, and the volume of the fistulous tract is determined. Stromal-vascular fraction and autoplasma enriched with platelets are prepared. Video gastroduodenoscopy is performed, the internal opening of the fistulous tract is visualized, into which the injection needle is inserted. A pre-prepared stromal-vascular fraction is injected into the fistula cavity in the amount that was determined during fistulography and is necessary to completely fill the entire fistulous tract. Using the same injector needle, a mixture is injected into the area of the internal opening of the fistula: stromal-vascular fraction and autoplasma enriched with platelets in a ratio of 1 to 1 at four points at the corners of the rhombus of the fistulous passage and a roller is formed that closes the internal opening of the fistulous passage.

EFFECT: method allows to accelerate the treatment of chronic fistulas of the upper gastrointestinal tract, improves the regenerative effect in the tissue surrounding the defect, reduces the number of complications, improves the patient's quality of life, does not require long-term training and high costs.
