METHOD OF RECONSTRUCTION OF PULMONARY VALVE IN TETRADE OF FALLOT WITH BILATERAL STRUCTURE Russian patent published in 2023 - IPC A61B17/00 

FIELD: medicine; cardiac surgery.

SUBSTANCE: with a bilateral structure of the pulmonary valve, the leaflet located anteriorly is cut off from the valve ring of the pulmonary artery (PA). The leaflet tissue is mobilized on half of its base, then the cut-off part of the leaflet is reimplanted, forming a neosinus with a neocommissure height corresponding to the level of the opposite commissure, while fixation is performed with a continuous monofilament suture along the proposed border of the neosinus. Then, a U-shaped suture is additionally applied, reinforced with spacers in the area of the neocommissure. Infundibulectomy and ventriculotomy are performed until the bougie of the calculated diameter can freely pass through the outlet section of the right ventricle, respectively, according to the body surface area and age. In the space free from the reimplanted part of the pulmonary leaflet, a leaflet from the autopericardium is implanted, after which the integrity of the PA is restored with a separate patch from the autopericardium performing plastic surgery of the ventriculotomy area and the trunk of the pulmonary artery.

EFFECT: method makes it possible to achieve adequate hemodynamic function of the valve, increase thromboresistance and wear resistance.
