FIELD: medicine; ophthalmology.

SUBSTANCE: phacoemulsification is performed in case of development of intraoperative syndrome of deviation of infusion flows and leakage of irrigation solution with fragments of lens masses into the retrolental space. A cohesive viscoelastic is introduced into the capsular bag, the volume of the anterior chamber is restored, and the capsular bag is straightened. The capsular bag is inspected, a fold of the posterior capsule is formed with a 30 G injection needle, and a linear capsulotomy 2 mm long is formed in the center of the posterior capsule. Posterior capsulorhexis 25 G is performed through a corneal tunnel incision with collet tweezers, implantation and positioning of the IOL in the capsular bag, evacuation of the viscoelastic from the anterior chamber and capsular bag. After revision of the capsular bag, confirming the absence of defects in the capsular bag and the presence of lens detritus in the retrolental space, a polymeric intracapsular ring is implanted using an injector. After performing the posterior capsulorhexis, drainage of the retrolental space is performed using the “push-push” technique. Irrigation fluid and lens fragments are evacuated from the retrolental space using a 27 G L-shaped cannula connected to a BSS balanced saline syringe. In a particular case for the revision of the capsular bag, a biconcave pusher spatula is used. In a particular case, the L-shaped cannula used for drainage of the retrolental space has a curved distal end 0.5 mm long.

EFFECT: method allows to increase the safety of surgical intervention, and reduce the risk of postoperative complications.
