METHOD OF DETERMINING A COMPLEX BIFURCATION LESION OF THE CORONARY ARTERIES Russian patent published in 2023 - IPC A61B6/02

Abstract RU 2802855 C1

FIELD: medicine; cardiology; cardiovascular and X-ray endovascular surgery.

SUBSTANCE: invention can be used to determine complex bifurcation lesions of the coronary arteries. Selective angiography is performed and the main criteria are identified: a true bifurcation lesion 1-1-1 and 0-1-1 according to the Medina classification, the degree of narrowing of the lumen of the lateral branch (BV) is 75% or more, the diameter of the lumen of the BV is 2.5 mm or more. Secondary criteria are also identified: the presence of acute or chronic occlusion of the BV, the extent of the BV lesion of 10 mm or more, the presence of the only significant BV in the basin of the main branch (BV), the length of the BV lesion of 30 mm or more, the presence of a thrombotic lesion, the bifurcation angle is less than 45° or more than 70°, the presence of moderate or severe calcification of the coronary artery, tortuosity of the coronary artery, the presence of trifurcation, the presence of percutaneous coronary interventions in history in the bifurcation projection. And in the presence of all three main criteria and three secondary criteria, a complex bifurcation lesion of the CA is determined.

EFFECT: method allows to assess a bifurcation lesion, starting from the degree of stenosis, ending with a general angiographic analysis of the entire coronary bed, by highlighting the main and secondary criteria that comprehensively characterize the degree of the bifurcation lesion, on the basis of which a conclusion is made about the presence or absence of a complex bifurcation lesion.
