INJECTION OF IMPLANTATION OF THE POSTERIOR LAYERED CORNEAL GRAFT Russian patent published in 2023 - IPC A61F9/07

Abstract RU 2803860 C1

FIELD: medical equipment.

SUBSTANCE: invention is intended for implantation by pulling a corneal graft during posterior layered (endothelial) keratoplasty with removal of the recipient's Descemet's membrane (DM). The injector for implantation of the posterior layered corneal graft includes a platform for the graft and a funnel. The injector is made of a biocompatible medical polymer, additionally contains a body, a rod connected to a pusher on one side and with a plug on the opposite end, a tip with a through channel inside. Inside the body of the injector, an oval-shaped cavity is formed, extending from the proximal end of the body to the site for the graft, which narrows in the shape of a trapezoid to the funnel. The width of the platform adjacent to the hull - the larger base of the trapezoid corresponds to the width of the hull. The width of the site at the point of transition to the funnel is the small base of the trapezoid, corresponds to the inlet wide part of the funnel, on the site, not reaching 4 mm to the entrance to the funnel, and continuing 1 mm into the channel of the funnel, a furrow is made along the longitudinal axis. The body length is 30-50 mm, width 7-16 mm. On the sides of the body there are notch stops designed for easy fixing of the injector. The rod is made straight, its surface is congruent to the surface of the cavity in the housing. The rod is installed inside the housing along its longitudinal axis of symmetry with the possibility of reciprocating motion. The plug is made in the form of a truncated oval cone, the small base of which is directed towards the funnel. The surface of the plug is congruent to the funnel channel, the proximal end of the rod is connected to the pusher. The graft platform passes into a funnel with a tip. The funnel is arcuately beveled to the site for the graft. The funnel has a channel tapering towards the tip with a cross-section in the form of an oval, the major axis of which at the point of transition to the platform is equal to the size of the small base of the platform. The funnel channel passes into the straight channel of the tip, which has an oval-shaped cross section, the major axis of which is perpendicular to the longitudinal axis of the injector and lies in a plane parallel to the graft site. Inside the tip channel, along its entire length, a rounded protrusion is made from above, directed to the longitudinal axis of the injector. The tip is beveled downwards at an angle of 35-60 degrees, with the formation of a tip, on which a longitudinal groove is made.

EFFECT: use of the invention makes it possible to reduce the intraoperative traumatization of the posterior layered graft at the stage of its implantation into the anterior chamber of the eye and, consequently, improve the clinical and functional results in patients in the postoperative period.
