SUBSTANCE: reconstruction of the male perineum, surgical removal of the cavernous bodies, bilateral orchiectomy, formation of a skin case from the skin of the penis with an inverted head, preservation of the dorsal neurovascular bundle and their subsequent introduction into the formed channel between the rectum and bladder, formation of the labia majora from marsupium tissues. At the same time, access to the pelvic peritoneum is performed laparoscopically. The peritoneum is dissected between the anterior wall of the rectum and the bladder. In the defect of the pelvic peritoneum, an inverted glans penis drawn from the perineum is identified, which is fixed to the pelvic peritoneum with separate sutures. After that, the peritoneum is sutured tightly.

EFFECT: method allows to maintain the length and adequate fixation of the skin sheath of the neovagina, to eliminate the need for long-term sexual rehabilitation by bougienage.
