FIELD: neurology; surgery; neurosurgery.
SUBSTANCE: intended for surgical correction of spasticity, representing a method of less invasive surgical access for selective dorsal rhizotomy. Muscle testing is performed to determine the level of access. A linear vertical incision is made at the selected level, 3-5 cm in size. The interspinous ligament is crossed. At the base of the overlying spinous process, a rectangular fragment 10-15 mm wide and 5 mm high is bitten out medially within the articular processes from the lower side. A fragment identical in size and shape is bitten out in the medial part of the base of the underlying spinous process within the articular processes from the upper side.
EFFECT: method provides improved functional outcomes of the operation, negates the risk of developing post-laminectomy syndrome by reducing tissue trauma as in the case of incision and at the same time providing access to the roots of the spinal cord, as well as the absence of the need to remove the spinous processes.
