METHOD FOR PREVENTION OF SECONDARY FIXATION OF SPINAL CORD IN CHILDREN WITH SPINAL DYSRAPHISM Russian patent published in 2025 - IPC A61B17/00

Abstract RU 2837014 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to neurosurgery, and can be used for prevention of secondary fixation of spinal cord in a child with spinal dysraphism. Posterior median approach is made at the level of a congenital bone defect of the vertebrae. Dura mater (DM) is opened, meningomyeloradiculolysis is performed. Then, a continuous suture is applied in the frontal plane with a non-traumatic needle with an absorbable suture, alternately suturing internal sheets of the dura mater on each side of the spinal cord. Each prick in and out is performed at 2 mm from the posterior surface of the spinal cord towards the free edge of the dura mater of the ipsilateral side. At first, the inner layer of the dura mater is punctured with the suture passing intradurally and externally on one side of the spinal cord. Then the suture is delivered parallel to the posterior surface of the spinal cord on the counter-lateral side, and the inner layer of the dura mater is pierced with the suture being drawn intradurally and externally. Distance between adjacent needle sticking points on each side is 15 mm. Suture length is equal to the length of the dysplastic spinal cord. When applying the suture, the suture tension is maintained without matching the sutured inner leaves of the dura mater. Edges of the dissected dura mater are sutured together.

EFFECT: method provides preventing secondary fixation of the spinal cord and its roots by creating a free space between the spinal cord and the dorsal surface of the dural sac.
