FIELD: medicine; bariatric surgery.
SUBSTANCE: after installing the trocars, a longitudinal resection of the stomach is performed. In the area of the staple line, the upper corner of the proximal part of the duodenum is sutured. A 350 cm long loop of the ileum is counted from the ileocecal angle. The duodenal stump is sutured to the ileum, thus forming the first row of the posterior lip of the anastomosis. A sequential dissection of the duodenum and small intestine is performed 1 cm from the line of the first suture. In the part of the anastomosis distal from the laparoscope, 1.5–2 cm from the end of the line of dissection of the duodenum and ileum, one of the needles of a double-needle thread is sutured and pulled to the middle of the distance between the needles. Using a continuous suture of one of the needles, the posterior lip of the anastomosis is formed, which ends 1–1.5 cm beyond the incision line. Then, using a second free thread needle, a continuous gray-serous suture is used to form the anterior lip of the anastomosis.
EFFECT: method allows to reduce the risks of complications and increase the efficiency of biliopancreatic bypass surgery by optimizing the surgical technique of duodenoile bypass surgery with one anastomosis with a narrow gastric sleeve and a total loop length of 350 cm in obese patients.
