FIELD: medicine; operative urology.
SUBSTANCE: digital processing of computed tomography data and construction of a 3D structure of the upper urinary tract is carried out using DIcom image processing programs for computers. 60 minutes before the X-ray examination, the patient takes a single dose of 80 mg of furosemide orally, after which the excretory function of the kidneys is assessed at the 7th minute of a computed tomography scan of the abdomen and pelvis with contrast, and the study is carried out at the time of filling the bladder until the patient has a physiological urge to urination. After digital data processing, a visual assessment of the ureter is carried out: the presence of deviations of the ureter is determined at three control points corresponding to physiological narrowings, namely: in the pyeloureteral segment, at the level of intersection of the ureter with the iliac vessels and in the juxtavesical section of the ureter, in the absence of significant deviations of less than 110° and ureteral strictures, and the width of the ureter at control points of more than 2 mm it is possible to perform transurethral intervention without dilation of the ureter.
EFFECT: method makes it possible to improve the visualization of the ureter, obtain a clear image of all structures of the upper urinary tract, and predict the possibility of unhindered passage of the endoscope and/or 12/14 Ch ureteral casing during transurethral and/or percutaneous interventions for nephrolithiasis.
