FIELD: medicine, diagnostic radiology, oncology.

SUBSTANCE: multiparametric ultrasound examination is performed in 3 stages. At the first stage, a study is carried out in B-mode and color Doppler mapping (CDC) and, in the presence or absence of specific signs of metastatic lesions of the axillary lymph nodes (ALN), a low or high risk of metastatic lesion of the ACL is determined, respectively. If there are nonspecific signs of metastatic lesions, a second stage of the study is carried out, in which two-dimensional shear wave elastography (2D SWEG) is performed with assessment of PLU staining according to three elastotypes. When determining the first elastotype, a low risk of metastatic lesion of the PLN is determined, when determining the third elastotype, a high risk of metastatic lesion of the PLN is determined, when determining the second elastotype, the third stage of the study is carried out. At the third stage, quantitative 2D SVEG data for PLU of the second elastotype are analyzed in the form of the EQI median stiffness indicator, and when the EQI median value is greater than or equal to 15.9 kPa, a high risk of metastatic lesion of the PLU is determined.

EFFECT: method increases the diagnostic accuracy of differential diagnosis of changes in the PLU in case of a suspicious formation of the mammary gland, makes it possible to identify the most changed PLU for biopsy, and optimizes the indications for multiparametric ultrasound using two-dimensional shear wave elastography.
