METHOD OF PERFORMING PHACOEMULSIFICATION OF COMPLICATED CATARACTS Russian patent published in 2023 - IPC A61F9/07 

FIELD: medicine; ophthalmology.

SUBSTANCE: phacoemulsification of complicated cataracts is carried out, two paracentesis, anterior continuous capsulorhexis, and phacofragmentation of the lens nucleus are performed using a phacoemulsifier. Paracenteses are formed at 9 and 15 o'clock position, respectively. A tunnel incision is made as the main access for inserting the phacoemulsifier tip, and a circular capsulorhexis is performed. It is performed at the stage of phacofragmentation, if it is impossible to carry out complete mechanical separation of the lens nucleus into fragments. Vacuum fixation is performed with the tip of a phacoemulsifier on one of the non-separated quadrants of the lens nucleus, with vacuum parameters of 450–500 mmHg, followed by removing the latter from under the edge of the capsulorhexis into the anterior chamber to the center of the capsulorhexis, and using a spatula or phaco-chop, making a horizontal movement towards the center of the pupil. The fragments are separated from the quadrant and phacoemulsified. Next, this manipulation is repeated with the remaining quadrants until the lens nucleus is completely removed.

EFFECT: method ensures safe separation of the dense lens nucleus into separate fragments that can be easily removed by emulsification in complicated cases.
