FIELD: medicine; surgery; herniology.

SUBSTANCE: longitudinal resection of the stomach is performed. The resected stomach is placed in a container to extract the macropreparation. From the abdominal cavity, the hernial sac is mobilized with its immersion into the abdominal cavity. The container with the resected stomach is removed from the abdominal cavity. The peritoneum is sutured using trocar approaches. The trocar is inserted in the left hypochondrium into the subcutaneous tissue, advancing it obliquely, from left to right, to the posterior layer of the rectus sheath. An additional trocar is installed in the paraumbilical area on the left, lateral to the epigastric vessels. Total extraperitoneal retromascular hernioplasty is performed with the installation of a mesh implant using laparoscopic access through skin incisions made at the stage of gastrectomy.

EFFECT: method allows for the simultaneous elimination of two diseases that aggravate each other at once — getting rid of morbid obesity and ventral hernia surgically during one operation with minimal surgical trauma eliminating the need to use expensive consumables; provides an increase in the therapeutic effect, a decrease in the risk of complications, a decrease in the intensity of pain and a reduction in hospital stay.
