FIELD: medicine; ophthalmology.
SUBSTANCE: phacoemulsification of cataracts is performed by implantation of an intraocular lens by simultaneous mechanical trabeculocleaning ab interno. After phacoemulsification of cataracts, under gonioscopic control, trabeculocleaning is performed through paracenteses using an instrument for mechanical trabeculocleaning, which consists of a handle, transition and working parts. The transition part consists of two elbows, the first elbow connected to a handle 13.5 mm long and the next elbow 15.5 mm long and 0.6 mm in diameter, decreasing to its distal part to 0.3 mm is located in the same plane and at an angle of 145°, and the working part is made in the form of a blunt round tip with diamond coating with a diameter of 0.33 mm is inserted with the distal end all the way to the inner wall of Schlemm’s canal and, under visual control, a “scraping” movement is made along an arcuate path, mechanically clearing the trabecula of pigment. In a particular case, if there is goniosynechia in angle-closure glaucoma, they are first separated with the distal end of the instrument for mechanical trabeculocleaning, followed by the instrument's impact on the inner wall of Schlemm's canal.
EFFECT: method allows to reduce intraocular pressure, achieve normalization of eye hydrodynamics in patients with open-angle, narrow-angle, closed-angle glaucoma and complicated cataracts, increase and stabilize visual functions, and reduce the probability of developing hemorrhagic complications in the postoperative period.
