FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to ophthalmology and can be used in treatment of glaucoma. Performed are: lens ablation, implantation of artificial intraocular lens and vacuum mechanical-hydrodynamic trabeculoplasty ab interno (MHTP ab interno), which lies in clearing trabecular zone from iris tissue with simultaneous increase, extension and deepening of anterior chamber angle by means of curved irrigation-aspiration cannula, which is introduced into anterior chamber through corneal tunnel cut, formed during crystalline lens ablation. After that, with continuous supply of irrigation liquid jet into anterior chamber and under visual control of value of anterior chamber angle, successively step-by-step, starting from the section, adjoining said cut, and further within the limits of not less than 0.95 of anterior chamber angle perimetre, by means of protruding beyond the cut of silicone tube of cannula with irrigation channel silicone tip of tube of cannula with aspiration channel carried out is, first, grip of iris stroma near its root by creation of negative pressure in aspiration channel after touching with above mentioned tip respective section of iris with further tension of iris tissue toward pupil centre till the moment of iris tissue tearing from above mentioned tip. After each passage of above mentioned section of anterior chamber angle perimetre direction of movement of irrigation-aspiration cannula is changed into opposite, and irrigation-aspiration cannula is removed from anterior chamber, if after another tension of iris tissue there are no visible changes of geometric parameters of anterior chamber angle, implantation of artificial intraocular lens being performed either before clearing trabecular zone from iris tissue or after it.
EFFECT: method makes it possible to simplify method of increasing anterior chamber angle in case of glaucoma, ensure maximal and stable in time opening of anterior chamber angle, maximal hypotensive effect, as well as atraumatic grip and tension of near-root sections of iris stroma.
