FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to therapeutic dentistry, and is intended for use in endodontic treatment of complex root system of masticatory group of teeth in diseases of the pulp, periodontal tissues, burdened by the presence of a transverse cavity connection of root canals. CBCT is used to determine the state of the root system of the masticatory group of teeth. Anaesthesia and isolation of the working field are performed using a surgical microscope. Carious cavities are prepared. If necessary, the fillings are removed and an endodontic access to the root canal orifices is created. Position of the root canal orifices is determined, and mechanical and medicinal treatment of tooth canals is carried out, determining the length of the root canals. Root canal visiography with introduced instruments is performed to locate 2 canals in one root on the visiographic image. After removing the instruments, two root canals in one root are filled with 3% sodium hypochlorite solution, providing a diagnostic aspiration test. When detecting a transverse cavity connection between canals with independent apical holes in one root, the entrance to the transverse cavity connection in the first canal is determined with the instrument. Transverse cavity connection is expanded on the side of the first canal, and the microbial flora is removed with an XP-endo Finisher. Two root canals in one root are again filled with 3% sodium hypochlorite solution, providing a diagnostic aspiration test from the side of the first canal, instrument is used to determine the entrance to the transverse cavity connection in the second canal, and the inlet opening of the transverse cavity connection of the second canal is expanded, and microbial flora is removed by the tool XP-endo Finisher. Lubricated layer is removed from walls of canals and transverse cavity connection using 17–20% solution of ethylenediaminetetraacetic acid and irrigation of canals and transverse cavity connection with 3% sodium hypochlorite solution with activation of ultrasound is performed. Canals and the transverse cavity connection are dried, the cannula of the syringe with the bioceramic sealer is introduced into the first canal and it is filled with a sealer from the apex to the transverse cavity connection, a cannula of a syringe with a bioceramic sealer is introduced into the transverse cavity connection through the first canal and the transverse cavity connection is filled with the sealer; the output of the sealer is visualized in the second canal. Cannula of a syringe with a bioceramic sealer is introduced into the second canal and filled with a sealer from the apex to the transverse cavity connection. Both canals are obturated in one root from the apex of the tooth to the transverse cavity connection with gutta-percha pins using the vertical condensation technique, transverse cavity connection is obturated with thermoplasticized gutta-percha, gutta-percha output is visualized in the second canal, canals are obturated from the transverse cavity connection to orifices with thermoplasticized gutta-percha. Control viziography of tooth and filling of crown part of tooth are performed.

EFFECT: method, due to effective penetration of disinfectant and filling material into all its canals, including transverse cavity connection with their subsequent obturation, allows recovering the functions of the affected tooth with a complex root system burdened by the presence of a transverse cavity connection between the canals in one root with independent apical holes.
