FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to dentistry, and is intended for use in endodontics in eliminating an iatrogenic factor in the form of a fractured fragment of an instrument stuck in a tooth canal. Computed tomography of the tooth is performed, and the position of the fragment of the filler to be removed is detected, and tissues are anaesthetized. Dental deposits are removed, rubber dam is installed, carious cavity is prepared under visual control with the help of microscope. Lost tooth walls are restored if necessary. Canal filler fragment and the tooth canals are visualized using a microscope. In the traversed and treated canals, in the multi-rooted teeth, root canal orifices are isolated with fragments of sterile fluoroplastic sealing material, blocking the possibility of migration of the canal-filler fragment in them during extraction. Endoactivator nozzle is introduced into the canal at the centre of the canal filler or on the side thereof, the canal and the cavity of the tooth are filled with 40% citric or 18% etidronic acid, and the solution is activated with the endoactivator. Then the endoactivator nozzle is removed, the canal is washed with distilled water, the endoactivator nozzle is again introduced into the canal at the centre of the canal filler or on the side of it, 5.25% sodium hypochlorite is introduced into the canal, the solution is activated with an endoactivator, and the canal is washed again with distilled water. Above actions are successively repeated by adding solutions and activating them with an endoactivator until the canal filler is loosened from being stuck in the canal. Canal filler is removed from the canal with forceps; a control visual examination is carried out.
EFFECT: due to activation of combination of used solutions in stated sequence, providing decalcification and decollagenization of dentin, which allows to weaken tight fit of stuck and fragmented instrument, method enables to extract the canal filler fragment from the root canal without negative consequences, such as formation of a step in the root canal, fracture of the instrument used for extraction, loss of considerable volume of dentin and thinning of root walls.
