SUBSTANCE: invention can be used for the treatment of osteonecrosis of the femur and tibia. Under arthroscopic and radiological control, tunnelization of the lateral and medial condyles of the femur and tibia is performed. From the lateral side, 10–15 mm from the border of the osteonecrosis focus, parallel to the joint space in the axial plane, at least three fan-shaped diverging channels are drawn to the medial cortical layer. Then, using a Kirschner wire, a skin puncture is made from the medial side at the midpoint of the medial condyle 15–20 mm away from the border of the osteonecrosis focus. At least three fan-shaped diverging channels are drawn parallel to the joint space of the knee joint in a fan-shaped manner in the axial plane to the lateral cortical layer. In the presence of osteonecrosis of the metadiaphysis of the tibia and femur, fan-shaped counter tunneling from the medial and lateral sides is performed on the distal metadiaphysis of the femur and proximal metadiaphysis of the tibia.

EFFECT: method ensures restoration of the structure and function of the limb with the possibility of rapid relief of pain due to the impact of a low-traumatic surgical intervention on the area of the pathologically altered subchondral area of bone necrosis.
