FIELD: medicine; radiation diagnostics.

SUBSTANCE: invention can be used for the diagnosis of idiopathic normotensive hydrocephalus (iNPH). MR images of the brain are obtained in the sagittal (Sag) and coronal (Cor) planes and are processed by software. A median slice is selected from among the Sag images. The contours of the brain are determined using a pre-trained neural network, as well as the positions of the anterior commissure (AC), posterior commissure (PC), dorsum sella (ST) and foramen of Monroe (FM). The maximum intracranial diameter (MSID) is calculated. A slice with FM is selected from among the Cor images, taking into account the previously determined position on the Sag images. The contours of the lateral ventricles of the brain (LV) and the third ventricle of the brain (TV) are determined using a pre-trained neural network on a slice with FM. The longest vertical distance (FHVD) between the superior wall of the lateral ventricle and the middle of the foramen of Monro (FM) on the coronal slice are calculated. The Zed Evans index z-Evans = FHVD/MSID is calculated. A slice from PC is selected from among the Cor images, taking into account the previously determined position on the Sag images. Using a pre-trained neural network, brain contours on a PC slice, as well as LV contours and areas of disproportionate expansion of the subarachnoid space in the Sylvian fissures and in the convexital parasagittal regions (DESH) are determined. On the coronal slice passing through the PC angle (CA) between the two walls of the ventricles, the maximum height (CMW) of the ventricle, the height (SVW) of the supraventricular medulla and the severity of DESH are calculated. The ratio BVR = SVW/CMW is calculated. The calculated values of the parameters z-Evans, CA, BVR and DESH severity are compared with reference values, at which a score from 0 to 2 points is assigned: for z-Evans <0.3 is 0 points, 0.3–0.35 is 1 point, > 0.35 is 2 points; for CA>95° is 0 points, 85–95° is 1 point, <85° is 2 points; for BVR >1.5 is 0 points, 1–1.5 is 1 point, <1 is 2 points; for DESH severity = 0 is 0 points, = 1 is 1 point, = 2 is 2 points. Based on the set of comparisons of parameters, a total score is determined and the probability of iGT is assessed: 4–8 points is high probability, 2–3 points is average probability, 0–1 points is low probability.

EFFECT: method provides automated diagnostics through software image processing and stated diagnostic criteria.
