SUBSTANCE: invention is related to a method for debranching the left renal and superior mesenteric arteries during resection of a thoracoabdominal aortic aneurysm (TAAA). The method for debranching the left renal and superior mesenteric arteries during resection of a thoracoabdominal aortic aneurysm, in which access to the thoracic and abdominal aorta is ensured with switching of blood flow in the visceral and renal arteries, reimplantation of the left renal artery is performed through an autovenous shunt, reimplantation of the superior mesenteric artery is performed through a synthetic shunt, in this case, an autovenous shunt and a synthetic shunt are anastomosed with the distal section of a pre-installed temporary aorto-iliac shunt, the aneurysm is resected and blood flow in the abdominal aorta is restored by removing the main part of the shunt while preserving the shunt section in the anastomosis zone of the transposed left renal and superior mesenteric arteries as permanent functioning prosthesis. The temporary aorto-iliac shunt is first anastomosed at one end through the lateral flake with a section of the descending thoracic aorta, and the opposite end is anastomosed with the left common iliac artery.
EFFECT: reduction of the risk of damage to the abdominal organs and the development of intestinal paresis, a reduction in the risk of damage to the duodenum and the development of intestinal-prosthetic infections, a reduction in the risk of developing postoperative pancreatitis, eliminating the risk of developing endoleaks, and avoidance of the need for a heart-lung machine.
