FIELD: medicine; dentistry.
SUBSTANCE: invention is intended for use during resection of the apex of the tooth root, cystotomy, cystectomy. The following is performed: a computer examination of the pathological formation, creation of a virtual model in STL format, 3D modeling of an individual template, production of an individual template, anesthesia of the operation area, incision of the mucous membrane in the periapecal area, folding of the mucoperiosteal flap, placement of a pre-made template with a window in the projection pathological focus. Using a computer examination, the topographic location of the infraorbital foramen on the anterior surface of the body of the upper jaw is additionally determined. 3D modeling of an individual template for infraorbital anesthesia and 3D modeling of an individual template with a window for creating bone access in the area of the planned operation are performed. Then templates are made, using a rubber stopper, the depth of immersion of the needle for conduction anesthesia in the area of the infraorbital foramen, obtained from 3D modeling, is measured. A template for infraorbital anesthesia is installed in the oral cavity, the needle is advanced along the axial channel of the guide hole until the rubber stopper stops, an aspiration test is performed, and if the aspiration test is negative, an anesthetic solution is injected into the area of the infraorbital foramen. After the onset of anesthesia, the mucous membrane is cut in the projection of the apex of the problem tooth, the mucoperiosteal flap is folded back, a template with a window is installed, and bone access is created.
EFFECT: due to preliminary planning of local anesthesia and surgical intervention the method makes it possible to increase the effectiveness of anesthesia of tissues in the surgical area, ensure that a larger volume of tissues is turned off from pain sensitivity and reduce the load of local anesthetic on the patient’s body.
