FIELD: medicine; pediatric surgery; urology.
SUBSTANCE: cystoscopy is performed using a 9.5 FG cystoscope, catheterization, balloon installation, pressure injection and balloon removal. Cystoscopy is performed with targeted retrograde pyelography through a 3 FG ureteral catheter using the Dinamic Angio program 15–30 frames per second and radiography up to 3 images per procedure on a GE innova installation, the extent of stenosis is determined. A balloon catheter is selected, after which the ureteral catheter is removed and the balloon is inserted along the guidewire. Positioning of the balloon is carried out under radiographic control. Then, using an insufflator, the atmospheric pressure is pumped up to 8 atm; after straightening the stenosis zone, installation of the balloon takes 60 seconds. Then the level of atmospheric pressure is reduced and the balloon is removed from the ureter, the parameters of the zone of ureteral stenosis are assessed before high-pressure balloon dilatation of the vesicoureteral segment of the vesicoureteral segment is performed. Next, a ureteral J-stent is installed along the conductor into the ureter with the retraction threads brought out and the conductor is removed with cystoscopic and fluoroscopic control of the stent position, stenting lasts 30 days, followed by removal on an outpatient basis.
EFFECT: method allows to perform intraoperative assessment of the intervention without ureterotomy, which makes it possible to accurately select a balloon of the required size, prevent bleeding, scarring, and preserve innervation and blood supply to the ureter.
