FIELD: medicine; pancreatobiliary surgery; gastroenterology.

SUBSTANCE: performing a pancreatic head resection with leaving a parenchyma layer along a medial edge of the duodenum, containing branches of anterior and lower pancreatoduodenal arteries, providing its blood supply. Segment of jejunum on a vascular pedicle is separated at distance of 50–60 cm from duodenojejunal junction 20–25 cm long, the proximal end of which is closed. Jejunal contents are sampled for bacteriological examination of the parietal and intraluminal flora on the proximal and distal portions with separate swabs. Distal end of the jejunum is anastomosed with an end-to-side duodenum by single-row separate sutures. Longitudinal pancreatojejunostomy is performed along a tail, a body and a wound cavity of the head of the pancreas. Bacteriological examination shows bacterial overgrowth and prescribes Rifaximin 400 mg 2–3 times a day.

EFFECT: method enables the patients to improve the quality of life, increase the level of working capacity, effectively relieve pain syndrome, enables to arrest the phenomena of exo- and endo-secretory pancreatic insufficiency, stop progression of chronic pancreatitis in postoperative period due to detection of parietal and intraluminal flora of jejunum and appropriate drug therapy.
