FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to endocrinology, anaesthesiology and resuscitation, and can be used for treating ketoacidosis in children of 10–14 years old with type 1 diabetes mellitus. A central vein is catheterized using a three-lumen catheter. Glucose level is continuously monitored through the proximal segment of the three-lumen catheter, acid-base state and blood gases through the middle segment of the three-lumen catheter, bolus introduction of 10–20 units of insulin solution is performed through the distal segment of the catheter, having previously filled the manifold coming from the dosing syringe, and the distal segment of the central venous catheter with the insulin solution. Then, continuous introduction of insulin solution is performed with the help of a dosing syringe through a distal segment of the central catheter at rate of 0.3–1.4 units/kg/h. At the same time, dose of 4% sodium bicarbonate solution, calculated according to the formula, is introduced into a peripheral vein to reach level of 4 mmol/l by BE with maintaining this level throughout the treatment.
EFFECT: method makes it possible to withdraw children 10–14 years old from the process of ketoacidosis with decompensation of type I diabetes due to the possibility of controlling the circulatory system by using a three-lumen catheter in the central vein for continuous measurement of blood glucose, as well as bolus and continuous administration of insulin solution with a dosing syringe, systemic monitoring of blood glucose, acid base balance and blood gases.
