FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to surgery and orthopaedics, and can be used for elongation of toe phalanx in brachyphalangia by introduction of autograft and blood plasma rich in growth factors (PRGF) into the osteotomy area. Osteotomy of proximal phalanx is performed. Phalanx diaphysis is separated at the osteotomy site and a transverse osteotomy is performed by orienting the saw blade parallel to the frontal plane of the phalanx at distance of 0.5 cm from the articular cartilage line of the metatarsophalangeal joint from the back to the foot sole, wherein saw blade is installed at angle of 90° to the horizontal axis of the metatarsal bone. Graft is sampled from a proximal phalanx of an adjacent finger. Diaphysis of the proximal phalanx is separated in layers. First osteotomy is performed at 0.5 cm from the border of the articular cartilage proximally, wherein the saw blade is oriented perpendicular to the proximal phalanx from the outside to the inside. One proceeds distally to the distance required for the graft; a second osteotomy is performed parallel to the first one, and a cylindrical graft is prepared, which is placed in a saline solution of 0.9% NaCl. Two segments of the diaphysis are set parallel to each other, and the phalanx osteotomy is temporarily fixed with 1 mm K-wire. A cortical layer is drilled with a cannulated drill to a chondral layer of a distal phalanx segment. Fragments are fixed by a cannulated self-drilling compression screw inserted along the wire. Graft is placed in the area of the performed osteotomy of the proximal phalanx of the shortened finger; the intraosseous introduction of PRGF is performed in the area of the graft insertion. Distal segment is moved to restore the coaxiality of the proximal osteotomy fragment, the distal osteotomy fragment and the graft. Transarticular osteotomy is fixed.
EFFECT: method provides higher effectiveness of toe phalanx elongation in brachyphalangia due to single-stage defect elimination with application of autograft and blood plasma rich in growth factors.
