FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to traumatology and orthopaedics and can be used in treating patients with insufficiency of the extensor apparatus of the knee joint with significant metaepiphyseal defects of the tibia at the stage of revision endoprosthesis replacement. Method involves making an individual implant according to computed tomography of the involved and contralateral bones. Implant is made of a porous structure in the form of a monoblock with a hole for a stem of the tibial component of the endoprosthesis with a tibial tuberosity having through channels for fixation of the autograft. Tendon of a long peroneal muscle is taken. Tendon is delivered through the preserved fragments of the patellar ligament, bypassing the patella medially, through its upper pole and laterally using a purse-string suture technique. Tendon ends are placed within the formed tuberosity of the individual implant. After the officinal components of the revision endoprosthesis are placed on the bone cement in the position of full extension and slight tension, the tendon graft is fixed on the formed tuberosity.
EFFECT: method enables providing the anatomically grounded fixation of the extensor apparatus and taking into account the force vector for the work of the quadriceps muscle of thigh, which influences the functional recovery of the extensor apparatus.
