SUBSTANCE: invention can be used for patellar ligament autoplasty. Two tendons of the long peroneal muscles are taken, two transverse channels are created in the middle and lower parts of the patella, grafts are passed through these holes with a drill, with a diameter equal to the diameter of the harvested tendons, so that equal parts of the grafts remain on both sides. At a distance of 5–10 mm medially and laterally from the tibial tuberosity, using a drill with a diameter of one and a half times the diameter of the tendons, two through holes are obliquely distally made with the passage of the opposite cortical bone. The ends of both tendons are passed through these holes in the position of extension in the knee joint, stretched with threads and fixed with screws with a diameter equal to the diameter of the holes.

EFFECT: increasing strength of graft fixation and efficiency of restoration of lower leg extension function in conditions of chronic failure due to the use of long peroneal muscle tendons.
