EXTENDED LOWER BLEPHAROPLASTY METHOD Russian patent published in 2024 - IPC A61F9/07 

FIELD: medicine.

SUBSTANCE: invention relates to ophthalmology, maxillofacial, plastic surgery. Horizontal incision of the skin along a lower eyelid is made 2 mm from the ciliary edge downwards, extending beyond an external corner of eye 10–15 mm, along a natural wrinkle, skin flap 10–15 mm wide is exfoliated downwards. A circular muscle of the eye is dissected horizontally along the fibers, 5 mm from top to bottom, and then—a tarzo-orbital fascia. Prolapsing orbital fat is resected. Orbital ligament (ORL) is transected along a lower edge of the orbit. Thereafter, tissues are exfoliated downwards along the surface of suborbicularis oculi fat—SOOF, to its lower border with intersection—zygomaticus cutaneous ligamentum—ZCL. Further, a fat flap SOOF on a lower base is mobilized along the periosteum, SOOF fat flap is displaced obliquely upwards and laterally fixed with 2–3 sutures to periosteum of external edge of orbit in stretched position. Excess fat flap is excised in the lateral section in the form of a triangle. In particular, the thickness of the fat flap SOOF is corrected by planar and lateral resection.

EFFECT: method allows to expand technical capabilities and effectiveness of blepharoplasty operations due to additional use of SOOF fat flap for reliable additional support and filling of sunken lower eyelids.
