FIELD: medicine.
SUBSTANCE: invention relates to angiology, vascular surgery, and can be used to select a surgical approach in patients with occlusive stenotic involvement of the internal iliac artery (IIA). For this purpose, preoperative angiography is performed, and patency of arteries in the IIA pool is determined in the following segments: a proximal segment of the IIA from the level of its orifice to the division into an anterior and a posterior shaft; a zone of division of the IIA into an anterior and posterior trunk, an anterior trunk to the level of its division into an internal genital artery (IGeA) and an inferior gluteal artery (IGlA), a posterior trunk of the IIA and an intrapelvic segment of the superior gluteal artery (SGA); intrapelvic segment of internal genital artery and inferior gluteal artery (if any). Obtained data are used to determine the type of occlusal-stenotic involvement of the arteries as follows: type 1—isolated hemodynamically significant, stenosis more than 75% or occlusion of one arterial segment in the IIA pool; type 2—widespread hemodynamically significant lesion of several arterial segments in the IIA pool with preservation of partial patency; type 3—occlusion of all arterial segments; depending on the type of occlusive lesion, the appropriate surgical approach is selected: in case of type 1—endovascular operation of balloon angioplasty of the affected area with stenting with one stent; in type 2—endovascular treatment with implantation of several stents after balloon angioplasty of involved arterial segments or open operation of extra-anatomical bypass of branch of IIA in gluteal region; type 3 involves an open extra-anatomical bypass of a branch of the IIA in a gluteal region.
EFFECT: method enables optimizing the surgical approach.
