METHOD FOR BIATREAL CRYOABLATION WITH SURGICAL OCCLUSION OF LEFT ATRIAL APPENDAGE BY COMPLETELY THORACOSCOPIC METHOD IN ATRIAL FIBRILLATION Russian patent published in 2024 - IPC A61B17/00 A61B18/04

Abstract RU 2826346 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to cardiovascular surgeons. An artificial circulation is formed by performing a right-sided thoracoscopic approach to the heart and performing peripheral cannulation of the femoral artery and vena cava. That is followed by creating conditions for forming a dry surgical field by occluding an ascending aorta and antegrade cardioplegia through the aortic root. Thereafter, cryoablation lines are applied through the opened cavities of the right and left atrium, after which the mouth of the left atrium is closed. When applying cryoablation lines, the cryoprobe is first positioned in the left atrium (LA) from the left angle of the incision to the orifices of the pulmonary veins. Further, the cryoprobe is positioned from the right angle of the LA incision to a point in the area of the left pulmonary veins orifices, to which the previous cryoablation line is brought, so that both lines are connected. Then a final cryoablation line is formed from the previously formed one to the fibrous ring of the mitral valve. In a particular case, cryoablation along each line is carried out for 20 minutes at T -60 °C.

EFFECT: method enables achieving high rate of surgical cure of AF with quality occlusion of left atrial appendage (LAA) with minimal traumatism of approach and eliminating disadvantages of analogues.
