METHOD FOR LEFT VENTRICULAR DRAINAGE IN PROSTHETIC REPAIR OF AORTIC VALVE FROM UPPER-MIDDLE MINI-J-STERNOTOMY Russian patent published in 2020 - IPC A61B17/00

Abstract RU 2726606 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, particularly to cardiovascular surgery. After cannulation of femoral vessels and beginning of artificial blood circulation (BC) through skin punctures in 3, 4 and 5 intercostal spaces, endoscopic ports are installed on the right side. Pericardium 1–2 cm above the diaphragmatic nerve is longitudinally exposed under direct visual control by means of an endovideosurgical system. Performed is dissection of epicardial fat in an interatrial sulcus. Purse-string suture is applied on the mouth of the right upper pulmonary vein. That is followed by a puncture in the purse-string suture in a left atrium cavity through which a draining cannula of diameter 12 to 16 Fr is introduced, and then it is delivered through the mitral valve into the LV cavity under the control of transoesophageal echocardiography.

EFFECT: method enables reducing the risk of overdevelopment and left ventricular injury in the early postischemic period, optimizing visualization of the aortic valve during the main stage of the operation.
