FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to surgery, vascular surgery, phlebology, and can be used for endovasal laser coagulation of saphenous vein in varicose veins of lower extremities, complicated by previous thrombophlebitis. Under ultrasound control, puncture of greater or lesser saphenous vein is performed, into the lumen of the vein to the area with the presence of parietal thrombotic masses, which prevent the laser light guide, laser light guide is introduced through angiographic catheter, tumescent solution is introduced, laser coagulation of vein is performed with light guide traction in distal direction. After visualization of the thrombotic mass boundary and with complete occlusion of the GSV or SSV, the tumescent solution is introduced extravasally in amount of 5 to 100 ml at a rate of introduction of not more than 200 ml per minute into the unaffected vein, then passing the light guide further along the saphenous vein and positioning it, not reaching 1.8 cm to the saphenofemoral or saphenopliteal fistula, then, from the puncture point to the positioning area of the working end of the light guide, an infiltration paravasal anaesthesia is performed with a tumescent along the saphenous vein, wherein the total flow rate of the tumescent solution is from 150 to 300 ml, then laser coagulation of saphenous vein is performed at radiation power of 6 W at wave length of 1,940 nm in continuous mode.
EFFECT: method makes it possible to speed up the procedure, to increase the clinical effectiveness and to improve the cosmetic result due to the absence of repeated catheterization of the saphenous vein by the second approach.
