FIELD: medicine; cardiology.

SUBSTANCE: first stage involves myocardial revascularization by percutaneous coronary interventions, in which coronary arteries with diameter of more than 2 mm and stenosis of more than 50% are treated, and for which 2- and 3-generation drug-releasing stents are used. At the second stage, angiography of the right and left renal arteries, celiac trunk and superior mesenteric artery, then catheter denervation is performed by multipolar radiofrequency ablation under intravenous sedation and through femoral approach. Through the introduced into the common femoral artery introducer 6F using a guide catheter, a Symplicity Spyral catheter is delivered alternately to the orifice of the celiac trunk, superior mesenteric, right and left renal arteries, then it is used to perform multifocal ablation exposure of proximal celiac trunk and mesenteric artery, the entire main renal artery trunk, as well as all daughter branches of the renal arteries with a diameter of more than 3 mm, followed by a final angiography.

EFFECT: method enables improving the results of the endovascular treatment of ischemic heart disease in the patients with type 2 diabetes mellitus and hypertension with reducing the incidence of unfavorable cardiovascular events.
