FIELD: medicine.

SUBSTANCE: invention relates to surgery, and can be used for minimally invasive retrograde temporary intramedullary rib fractures. Approach is performed in the area of fractures of the rib bodies; the fracture markers are determined intraoperatively by means of an ultrasonic examination or by using an electro-optical converter. Layer-by-layer approach to the fracture site is 5–7 cm long, separating from this incision the point of fractures of 3–4 adjacent ribs. Highlighting the rib fracture intramedullary into the anterior fragment, using a wire drill, K-wire is delivered 1.5×250 mm with a sharp end until the wire, passing to the rib curvature and resting against it, perforates the anterior cortical layer, passes through soft tissues and comes out onto the skin surface. Wire is brought along the canal until the opposite end of the wire is at the fracture point at the anterior fragment; after manual reduction of fragments under visual control, the wire is delivered retrogradely into the posterior fragment of the rib fracture as far as it will go. Wire migration is prevented by shortening the non-implanted portion of the wire at distance of 2 cm from the skin surface and bending it at distance of 0.5 cm from the end. Wound is closed in layers and an aseptic dressing is applied.

EFFECT: method provides reduction of injuries, fixation of movable fragments of ribs, restoration of the chest frame, respiratory volume, early activation of patients due to peculiarities of rib fixation.
