METHOD FOR SURGICAL CORRECTION OF PELVIC FLOOR MUSCLE INSUFFICIENCY Russian patent published in 2024 - IPC A61B17/42

Abstract RU 2830200 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to surgical gynaecology. After preliminary hydrodissection, perineal skin and mucous membrane of posterior vaginal wall is incised to fornix using dissection scissors, anterior wall of rectum is suprafascially separated to edges of unchanged rectovaginal fascia, inner surface of pubic-rectal and pubic-coccygeal muscles from both sides. Thereafter, a diastasis between the bellies of the pubic-rectal and pubic-coccygeal muscles is determined visually and palpably; at least two separate ligatures of a polyfilament reabsorbed suture are applied on the bellies to eliminate the diastasis. Performing visual and palpatory control of recovery of muscular "plateau" and integrity of presenting rectal wall, thereafter, the rectovaginal fascia defect is repaired with a continuous blanket suture with a polyglycolide reabsorbable suture, and the medial portions of the puborectal muscles in the lower one-third of the vagina and in the centre of the perineum are sutured with a polyfilament reabsorbable suture material. Then vaginal mucosa is restored with continuous suture with polyfilament reabsorbed suture material, and perineoplasty is performed, vagina is treated with water solution of antiseptic and tampon with gauze tampon impregnated with iodine, which is removed from vagina 24 hours after operation.

EFFECT: method enables the anatomical recovery of the levator plateau, recovery of the puborectal muscle trophism, and therefore the qualitative recovery of the muscle function, which improves the functional outcome of the surgical treatment of pelvic organ prolapse, and significantly increases the effectiveness of the pelvic dysfunction correction.
