SUBSTANCE: invention refers to medicine, namely to surgery. During first 72 hours from the onset of the disease, an endoscopic intervention is performed using endo-ultrasonography and pancreatography. Localization of the defect of the wall of the major pancratic duct (MPD), the length of the defect of the wall of the MPD, as well as the area of pancreatic necrosis are determined. A multi-perforated pancreatic endoprosthesis is selected, which is used to stent the MPD. If observing necrosis within the head and isthmus of the pancreas, the MPD is stent with a straight or, in case of nonlinearity of the duct, a curved pancreatic multi-perforated endoprosthesis with diameter of 5Fr, 4-7 cm long. If observing necrosis in the region of the head, isthmus and proximal portions of the pancreatic body, the MPD is stent with a straight pancreatic multi-perforated endoprosthesis of diameter 5Fr or 7Fr, length 12-15 cm.

EFFECT: method enables minimally traumatic endoscopic intervention in patients with acute destructive severe pancreatitis in the early phase of the disease and signs of separation of the main pancreatic duct in the proximal and distal portions of the pancreas according to reasonable indications with minimization of risks of complications development in strictly regulated time of intervention.
