FIELD: medicine.
SUBSTANCE: invention relates to maxillofacial and plastic surgery. In the region of the upper lip on the side of the cleft, a scar-altered skin flap is cut out with two incisions, which gradually widens towards the base of the vestibule of the nose. Internal incision is extended along the edge of the membranous part of the nasal septum with the transition to the wing of the nose and excision of the crescent-shaped section of skin. External incision is made in the nasal cavity along the mucous membrane of the nasal septum behind the medial crus of the greater alar cartilage, reaching the level of its middle. Skin of wing of nose and tissues between medial pedicles of greater alar cartilages are mobilized. Endonasal incision is made on the healthy side. Skin of wing of nose is detached and mobilized. Partial resection of a lateral crus of a greater alar cartilage is performed on a healthy side. Then the resected cartilage fragment is moved and fixed to the arch of the greater alar cartilage on the side of the cleft. Thereafter, the arches of the greater alar cartilages are sutured together. Then, in the area of the wing of the nose on the side of the cleft from the edge of the pear-shaped opening along the protruding into the nasal cavity of the anterior part of the lateral leg of the greater alar cartilage, a mucous membrane is incised for up to 6.0 mm, from the lower edge of which the mucous membrane is incised at angle of 60° in the direction to the wing curl, forming a triangular mucosal flap. Further, the triangular flap is mobilized and rotated upwards, and along its upper edge, a second triangular mucous-cartilage flap is cut out. Then the triangular flaps are mutually moved and sutured to form a dome. Formed mucous-cartilage complex of inner wing of nose is fixed to arch of greater alar cartilage of healthy side with formation of dome in area of wing of nose.
EFFECT: method enables eliminating the flattening of the wing of the nose and preventing the postoperative deformation of the tip of the nose after unilateral cheiloplasty.
