FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to reconstructive maxillofacial surgery. Method for elimination of combined defects of the upper jaw using an individual endoprosthesis involves replacing the endoprosthesis of the defect after resection of the involved bone structures. Endoprosthesis is made on the basis of helical computed tomography of the facial skeleton individually for each patient using 3D printing by layer-by-layer fusion. It is sterilized, installed and fixed in the defect zone after its preparation. Preparation includes removal of soft tissues around bone edges, alignment of bone edge of defect by milling of bone edges of wound, checking the accuracy of contact of all attachment points of the endoprosthesis with the bone edges of the defect, formation of holes in the bone edges of the defect and fixation of the endoprosthesis in them by means of titanium mini-screws. If there is no endoprosthesis flotation under load, it is covered with a free revascularized soft tissue flap. Reconstructive material used is polyvinylidene fluoride with a porous architecture represented by interconnected spherical micropores with diameter of 300 to 900 mcm and wall thickness of 100 to 300 mcm. Density of filling during printing is from 30 to 75%, and from external side the defect zone is covered with skin-musculoskeletal peroneal flap.
EFFECT: improved functional results with reduced rate of complications caused by excessive traumatisation, more adequate restoration of frame function of the resected upper jaw fragment and improved quality of life of patients.
