FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely otorhinolaryngology. Before the operation, according to the computed tomography of the temporal bones of the patient with three-dimensional modelling, the dimensions and volume of the putative resected portion of the posterior bone wall of the external auditory canal are measured. On the model, four cutting lines are marked along the length of the auditory meatus to form a V-shaped edge of the bone resection on the side of the attic and an L-shaped edge of the bone resection on the side of the facial nerve. Long arm of the L-shaped edge is located above the facial nerve. During the operation, a retroauricular approach is used to inspect the tympanic cavity and atticoantromastoidectomy. Posterior bone wall of the external auditory canal is mobilized and resected on the lines previously planned by computed tomography with the help of a piezoelectric saw with a frequency of piezoelectric ultrasonic vibrations of 28–36 kHz and range of 60–200 Hz. Thickness of the working part of saw is 0.25–0.50 mm. Further, the middle ear and mastoid cavities are sanitized. Previously extracted fragment of the posterior wall of the external auditory canal is repositioned and the cutting line is reinforced with bone chips. At the final stage of the operation, the sound-conducting system of the middle ear, tympanoplasty, tamponade of the external auditory canal with a haemostatic sponge are performed. Postoperative wound is closed behind the ear.
EFFECT: method enables sanitation of the chronic infection focus and complete reconstruction of the posterior wall of the ear canal with restoration of the auditory function, considerably increasing the effectiveness of the surgical treatment of chronic suppurative otitis media.
