METHOD FOR CARRYING OUT PRIMARY PLASTIC REPAIR OF MAMMARY GLAND IN SURGICAL TREATMENT Russian patent published in 1997 - IPC

FIELD: medicine. SUBSTANCE: method involves executing subcutaneous mastectomy through para-alveolar cut continued into lateral quadrant to remove one block of the nipple and areola together with mammary gland tissue, fat, a portion of musculus pectoralis minor and lymphatic nodes. Cellulocutaneous flap with aponeurosis on the musculus rectus abdominis is cut out on the anterior abdominal wall, subcutaneous tunnel is formed from the abdominal wall to the zone of the mammary gland to be repaired. The cellulocutaneous flap is tunnelled and fixed on the frontal abdominal wall. Median portion of the flap is cut off to use it later as coverage for aponeurosis defect after carrying out de-epithelialization. Areola is marked on the rest portion of the flap. This part is de-epthelialized and region on the skin corresponding in size to the removed areola is left. Three-leaflet cut is carried out in the central zone to form nipple from the rest of the skin, de-epithelialized portion is covered with skin retained after executing subcutaneous mastectomy in forming the mammary gland. The aponeurosis is sutured and sutures are applied on the skin wound. EFFECT: improved functional results. 3 cl, 6 dwg
