SUBSTANCE: document is made in the form of a book and contains information certifying personality of its holder and medical information on state of his health. document has sheets in which results of medical examinations of holder are put dow, including laboratory and X-ray examinations, notes on passage for work from one enterprise to another one, results on taking of exams on minimum of sanitary knowledge, and physician's conclusions on admittance to work according to results of medical examination. A great number of vertical and horizontal lines forming a grid are applied onto main part of sheets, records are made in grid columns.
