FIELD: medicine, plastic surgery. SUBSTANCE: polyethylene film is preliminarily applied upon the present defect to cut out a template by its contour. Cutaneous incision is made along an anterior edge of the broadest muscle of back and own fascia of serratus muscle of back by not crossing the vascular stem to disconnect it by blunt and sharp way with subcutaneous-adipose fiber and serratus muscle of back upon a section corresponding to the defect size; in subcutaneous-adipose fiber above the broadest muscle of back it is formed a pocket-like cavity which corresponds in its size to a polyethylene template of the present defect. Then isolated fascia is unfolded in dorsal direction and placed inside the pocket with its external surface towards the inner surface of the skin. It should be at a maximum unfolded and fixed; between the fascia of serratus muscle of back and the broadest muscle of back a polyethylene template is driven and the wound is tightly fastened. After intergrowth of fascial vessels into above-situated skin, by a previous incision one should make an access to the broadest muscle of back and cut out a cutaneous-fascial fragment with vascular stem at the sizes corresponding to the present defect into which area it is replaced. EFFECT: more pronounced clinical effect at minimal cosmetic and functional defect in recipient and donor areas. 1 ex
