FIELD: medicine, neurosurgery. SUBSTANCE: one should apply transnasal-transphenoidal access to carry out a wide lancing of sella turnica. After trephination of sphenoidal sinus it is necessary to form a rigid "port" to introduce an endoscope and manipulators. For this purpose one should apply a nasal dilator along wound canal into the cavity of sphenoidal sinus to be fixed in wound canal by applying a dilator's distal end beyond osseous edges of trephinated opening. Tumor should be removed successively: at first, from the cavity of sella turnica by using rectangular view of operational field under the control with a microscope, then - posterior-superior departments of supracellulary part and laterocellulary tumor part under the control of rigid endoscope with oblique optics of 30 deg. Then one should remove anterior-superior departments of supracellulary tumor part under the control of rigid endoscope with oblique optics of 70 deg. The method enables to decrease traumaticity of operation due to providing complete visual control of all operational stages. EFFECT: higher efficiency of operation. 3 ex _
